Consent to Telehealth Services

This consent describes telehealth care delivered through the Row Telehealth platform by independent, licensed medical providers. Please read it carefully; you accept it when you submit a health assessment.

What telehealth is

Telehealth means receiving medical evaluation and care remotely, using electronic communications — such as secure questionnaires, messaging, phone, or video — instead of an in-person visit. Your provider may use the information you submit, your responses to follow-up questions, and any records you share to evaluate and treat you.

Expected benefits

  • Access to licensed providers without travel or waiting rooms.
  • Convenient follow-up and messaging with your care team.

Possible risks

  • Telehealth does not include a physical exam; your provider's evaluation is limited to the information you provide. Incomplete or inaccurate information can affect clinical judgment.
  • In rare cases, technical failures may delay evaluation or communication.
  • As with any transmission of information, security protections cannot guarantee absolute privacy, although we use safeguards required by law.

Your rights

  • Telehealth is voluntary. You may withhold or withdraw consent at any time without affecting your right to future care.
  • You may ask your provider about in-person alternatives at any time.
  • You may request a copy of your records from your treating provider.
  • Some states may require a synchronous (video or phone) visit before certain prescriptions; where required, this will be part of your evaluation.

Your responsibilities

  • Provide accurate, complete, and current health information.
  • Tell your provider about all medications, allergies, and conditions.
  • Seek in-person or emergency care when advised, or when your symptoms warrant it. For emergencies, call 911.

Prescriptions

No prescription is guaranteed. Prescriptions are issued only when a licensed provider determines treatment is medically appropriate. You are free to fill any prescription at a pharmacy of your choice.

Acknowledgment

By submitting an assessment through Row Telehealth, you confirm that you have read and understood this consent, that you are at least 18 years old, and that you consent to receive telehealth services from independent licensed providers accessed through the platform.
