Wegovy® and Zepbound® are FDA-approved for chronic weight management. Ozempic® and Mounjaro® are FDA-approved for type 2 diabetes and would be prescribed off-label, at your provider’s discretion.

How your dose is titrated
GLP-1 dosing is a ladder, not a single number. You begin at the lowest step and move up only when your provider decides the previous step is tolerated. These are the step schedules our pharmacy partner supplies for the compounded products.

Read these tables as a plan, not a promise. Dosing always starts at the lowest step and moves up only as tolerated. Your provider sets your step and adjusts it at check-ins based on how you are responding and what side effects you are having. Not everyone moves through every step, and staying at a lower step is a normal outcome.
Depending on which partner pharmacy fills your prescription, these compounds are co-formulated with vitamin B6 (pyridoxine) or vitamin B12 (cyanocobalamin). Compounded formulations are not FDA-approved. Oral troche milligrams are not comparable to injection milligrams. The two are absorbed differently, so do not read across the tables.
Which option is right for you?
Semaglutide
The most-prescribed starting point. Semaglutide is a GLP-1 receptor agonist that helps regulate appetite and slow stomach emptying, so you feel full sooner and stay satisfied longer. Your provider sets the dose and adjusts it at check-ins.
- Dosing Weekly subcutaneous injection. Started low and titrated up over several months
- Best for First-time GLP-1 patients
- Common side effects Nausea, constipation, diarrhea, reduced appetite, usually strongest during dose increases
- Not for you if Personal/family history of medullary thyroid carcinoma or MEN 2, history of pancreatitis, pregnancy
Tirzepatide
A dual-action option: tirzepatide activates both GIP and GLP-1 receptors. Often chosen by patients who plateaued on semaglutide or want a dual-pathway option.
- Dosing Weekly subcutaneous injection. Titrated up gradually across six steps
- Best for Patients who plateaued on semaglutide, or whose provider recommends a dual-pathway option
- Common side effects Nausea, constipation, diarrhea, reduced appetite. The same GI profile as semaglutide, usually strongest during dose increases
- Not for you if Same exclusions as semaglutide (MTC/MEN 2, pancreatitis history, pregnancy)
Oral semaglutide and oral tirzepatide
The same two molecules in a troche that dissolves in the mouth once a day, no needles, no refrigeration, easy to travel with. A good fit if injections are a dealbreaker. Oral absorption works differently from an injection, so the milligram figures are not comparable to the injectable ladders. Your provider will set expectations accordingly.

- Best for Needle-averse patients. Frequent travelers. Anyone without reliable refrigeration
- Common side effects Same GI class effects as the injectables, plus possible mouth or throat irritation
- Not for you if Same exclusions as the injectable GLP-1s
Wegovy®, Zepbound®, Ozempic® and Mounjaro®
Prefer the FDA-approved brand products? Your membership covers the provider relationship, check-ins, and care-team support. The medication is filled at a retail or manufacturer pharmacy and billed by that pharmacy. Wegovy® (semaglutide) and Zepbound® (tirzepatide) are FDA-approved for chronic weight management. Ozempic® and Mounjaro® are FDA-approved for type 2 diabetes. A provider may prescribe them off-label only where they judge that appropriate for you.
- Dosing Weekly pen injection. The manufacturer’s own titration schedule applies, not the compounded ladders above
- Best for Patients who want an FDA-approved product, or whose provider determines a brand product is the appropriate choice
- Availability Subject to change with manufacturer supply. Which product is prescribed is your provider’s decision
